Arthritis Treatment: Climbing the Ladder of Care

When someone comes into my office with arthritis in their hip or knee, one of the first conversations we have is about the different ways we can manage it. Arthritis doesn’t always mean surgery — in fact, most people start with simple treatments and never need to climb all the way to the top.

I like to think of it as a ladder. Each step represents a different level of treatment. We always begin at the bottom, and only climb higher if symptoms continue to interfere with daily life. Not everyone needs to reach the last rung, and what works for one person might not work for another.

Sometimes the most powerful changes are also the simplest. Staying active, pacing activities, and shedding even a few pounds can reduce the stress on joints. Many patients are surprised by how much relief they feel from just small adjustments in routine.

Step 2: Medications

Over-the-counter anti-inflammatories, like ibuprofen, or prescription-strength medications can calm down flare-ups. These don’t “fix” arthritis, but they can make daily activities more manageable.

Step 3: Therapy, Exercise, and Bracing

Strengthening the muscles around the joint is one of the best defenses against arthritis pain. Physical therapy programs or home-based exercises can improve stability and function. In some cases, a simple brace helps shift pressure away from painful areas, making it easier to move confidently.

When lifestyle changes and therapy aren’t enough, injections can provide relief. Options include cortisone, hyaluronic acid (HA), and platelet-rich plasma (PRP). Each has different benefits, and the right choice depends on your symptoms, activity level, and overall health.

Step 5: Surgery (When Needed)

Joint replacement is reserved for when all the other steps aren’t enough. It’s the last rung on the ladder — but for many people, it’s the one that restores mobility, reduces pain, and gets them back to enjoying life.
